About Job

The Patient Care Referral Coordinator is responsible for managing and coordinating incoming patient referrals to PDI for surgical scheduling. Additionally coordinating patients with specialists /other healthcare providers as needed. The Referral Coordinator performs a wide range of administrative duties to include referral processing, patient registration, verification of benefits, coordinating insurance authorizations, patient appointment scheduling, and communication with referral sources.

ESSENTIAL FUNCTIONS

  • Timely processing of incoming patient referrals. Ability to process 200 referrals per month.
  • Maintain and build positive relationships with referral partners and patient custodians.
  • Responsible for tracking, coordination and communication of patient referrals.
  • Ensures initial accurate patient registration, including demographics and current insurance information. Communicating with billing about pre-auth/eligibility needs.
  • Manages third party referral requests, coordinates collection of relevant medical and insurance information.
  • Point person for specialty medical reports for nurse and provider evaluation of medical records for qualifying patient as safe to treat in the ASC.
  • Collaborates in filling surgery schedule with 12-13 qualified patients per surgery day.
  • Assembles information concerning patient’s clinical background and referral needs
  • Confirms with billing and front office team that verification of benefits and prior authorization approval requirements are being requested and met.
  • Reviews details and expectation of referral with the potential patient.
  • Assists patients in problem solving potential issues related financial or social barriers (e.g., request interpreters as appropriate, transportation services, etc.).
  • Communicates patient updates between program and referring source.
  • Provide translation assistance in clinic for Providers and Nurses.
  • Assisting at the front desk when needed to ensure full coverage and patient confirmations.

SKILLS, ABILITIES, QUALIFICATIONS

  • High school diploma or equivalent; a post-secondary degree or relevant certification in healthcare administration may be preferred.
  • Bilingual fluency (Spanish/English).
  • Basic understanding of commercial and Medi-Cal related insurance eligibility requirements.
  • Polished, professional phone / interpersonal skills and email communication capability.
  • Previous experience in a medical office or healthcare setting.
  • Knowledge of medical terminology, healthcare procedures, and insurance practices.
  • Familiarity with electronic health record (EHR) systems and medical software.
  • Strong organizational and multitasking skills, with good judgement, effective prioritization.
  • Detail-oriented, with a focus on accuracy and patient care.
  • Ability to work independently and as part of a healthcare team.
  • Strong customer service focus.
  • Resourceful problem-solving skills with good judgement.
  • Accountability to complete delegated tasks and engage teamwork to complete items on time.
  • Able to sit at computer and keyboard type for 8-hour daily shifts.
  • Ability to lift up to 25 pounds
  • Able to see at typical near and far distances to accomplish clerical tasks.
  • Knowledge to operate standard office equipment: phones, copier, scan, fax, computer.

Must be Bilingual in Spanish
