Position Highlights
- Position: Social Worker Lead
- Location: Evanston Hospital- Cancer Center
- Full Time/Part Time: 40 hours
- Hours: M-F 8:30-5pm
- Required Travel: yes -local
- Conduct initial patient screenings and determine need for high-risk assessments.
- Complete comprehensive psycho-social and financial assessments, and provide supportive counseling for patients and families.
- Develop, implement, and update individualized care plans in coordination with the interdisciplinary team.
- Facilitate care transitions, including discharge planning, hospice referrals, and access to community resources (e.g., housing, food, transportation).
- Actively participate in performance improvement initiatives and contribute to refining workflows and service delivery.
- Utilize technology to document care, communicate with patients and teams, and track case management outcomes.
- Provide education and support around advanced care planning, self-management tools, and guardianship processes.
- Supervise and support clinical staff development, including training, onboarding, clinical competency evaluation, and team facilitation.
- License: LSW or LCSW license required
- Education: Master’s Degree from an accredited school of social work required
- Certification: CPR BLS Certification required
- Experience: Two years’ post-graduate experience preferred; healthcare and/or mental health experience preferred
- Skills: Experience with Electronic Medical Record (EMR) platform preferred. Experience with MS Office.
