Overview
The Hospice Social Worker provides compassionate, patient-centered psychosocial care to terminally ill patients and their families. This role supports individuals and families facing end-of-life issues, ensuring their emotional, social, and practical needs are met in alignment with the patient’s care goals and hospice philosophy. The social worker is an integral part of the interdisciplinary team and collaborates closely with nurses, physicians, chaplains, and volunteers to provide holistic care.
Responsibilities
- Conduct thorough patient assessments to identify needs and develop appropriate care plans.
- Provide direct patient care and support, ensuring a holistic approach to well-being.
- Maintain accurate medical documentation and medical records in compliance with regulations.
- Collaborate with healthcare professionals in group therapy settings to enhance patient outcomes.
- Advocate for individuals with disabilities, helping them access necessary resources and services.
- Offer home community care services, supporting patients in their living environments.
- Utilize knowledge of medical terminology to communicate effectively with healthcare teams.
- Engage with pediatric patients and their families, providing tailored support based on developmental needs.
Qualifications
- Degree in Social Work from an accredited institution required.
- Licensure minimum of LCSW in Texas is required or eligible to obtain.
- Minimum of 1-2 years of experience in healthcare, hospice, palliative care, or related field preferred.
- Strong interpersonal skills and the ability to work compassionately with diverse individuals and families.
- Knowledge of community resources, end-of-life care, and grief support practices.
- Ability to manage emotional intensity and maintain professional boundaries.
- Valid driver’s license and reliable transportation.
