Job Summary

This paid externship offers practical experiences for Social Work students in collaboration with LMSW preceptors. The Social Work student will develop a greater understanding of the professional component of the Social Work role as well as being the coordinator of patient care. He/she will also develop skills and confidence in performing a multitude of technical procedures. Works under the direct supervision of the Department Leadership and assigned preceptor. Strives for superior performance by consistently providing a product or service to leadership and staff that is recognized as ultimately contributing to the patient and family experience. Recognizes and demonstrates understanding of patient and family centered care.

Primary Job Responsibilities

  • Demonstrates a focus on the goals and objectives of the Obstetrics focused Social Work responsibilities as listed in Exhibit A.
  • Coordinates the integration of social services/case management functions into the patient care, discharge, and home-planning processes with other hospital departments, external service organizations, agencies, and healthcare facilities.
  • Responsible for monitoring the discharge planning process to ensure timely and efficient discharges are facilitated within the scope of the team.
  • Performs, documents, and reports any changes observed in condition or behavior of patient(s).
  • Document patient interventions in electronic medical records (EMR) computer terminal.
  • Attends in-service education programs as assigned.
  • Demonstrate job related aspects of patient safety to assure a safe environment, safe and effective use of equipment and technology, and decreased risk of potential adverse health outcomes for patients served.
  • Demonstrate knowledge of and support hospital mission, vision, value statements, standards of care, policies and procedures, operating instructions confidentiality statements, corporate compliance plan, standards of behavior, customer service standards, and the code of ethical behavior.
  • Must be able to demonstrate the knowledge and skills necessary to provide care appropriate to the age of patients served on his/her caseload.
  • Must demonstrate knowledge of principle growth and development over the lifespan and possess the ability to assess data reflective of the patient’s status and interpret the appropriate information needed to identify each patient requirements relative to his/her age-specific needs, and to provide the care needed as described in the unit’s and division’s policies and procedures.

JOB SPECIFICATIONS

EDUCATION

  • Must be actively participating in and within one semester of graduation from a nationally-accredited Masters in Social Work program.
  • Current certification American Heart Association BLS for Healthcare provider certification or obtain within 2 weeks of hire.

Essential Physical Abilities/Motor Skills

  • Able to travel independently throughout all Memorial Healthcare inpatient and outpatient facilities.
  • Small motor skills required for operating modern computer, office, and telephone equipment as utilized by Memorial Healthcare (MHC).

Essential Technical Abilities

  • Proficiency using modern office, computer and telephone equipment as used by Memorial Healthcare.
  • Ability to have hand-eye coordination and fine-motor skills.

Essential Mental Abilities

  • Ability to adapt and maintain focus in fast paced, quickly changing or stressful situations.
  • Ability to read and interpret a variety of documents including, but not limited to policies, operating instructions, white papers, regulations, rules and laws.
  • Ability to follow directions.
  • Ability to document findings and interventions as appropriate.

  • Able to see for the purpose of reading information received in formats including but not limited to paper, computer, reports, bulletins, updates, manuals.
