Job Description:
Duties/Responsibilities
- Manage a caseload comprised of medically and culturally diverse inpatient and outpatient families
- Assist families and patients by completing biopsychosocial assessments, identifying appropriate resources and referrals, assisting with discharge and transition planning, and serving as a liaison and an advocate between the patient and various systems
- Demonstrate knowledge of child development related to diagnosis, assessment, intervention, and referral of infants, toddlers, pre-school and school age children, and adolescents to appropriate community agencies
- Meet the needs of the families as identified, presented, or requested by the interdisciplinary team or physician
- Document information in the medical record consistent with departmental standards
- Maintain a working knowledge of a variety of referral sources and community resources appropriate for the patient population within the state of Texas
Required Skills/Abilities
- Bachelor's degree or Master’s degree in social work from an accredited (CSWE) Social Work Program required
- Texas State Board of Social Work Examiners (TSBSWE) licensure LBSW /LBSW-IPR or LMSW required
- Minimum 1 year of experience in health care social work preferred
- Position requires frequent use of telephone, fax, copy machine and computer
