Job Duties:
· Perform numerous activities on behalf of infants and toddlers with developmental delays and their families to assist the with accessing generic and specialized services and supports that promote independence, productivity and community inclusion
· Case planning, including facilitation and participation in the development, review and implementation of Individualized Family Service Plan (IFSP) within designated time frames
· Accept and process referrals in a timely manner and coordinate the performance of evaluations and assessments
· Conduct a Voluntary Family Assessment using a Routine Based Interview and ECO Map after consultation with family regarding concerns, strengths and priorities as it relates to the family’s daily routine
· Facilitate and participate in the development, review, and evaluation of the Individualized Family Service Plan (IFSP), inclusive of Six-month reviews, Annuals Reviews, and Transition Plans
· Serve as a team member with other staff, therapists, and service providers
· Maintain records and documentation in a timely and organized manner in compliance with AEIS standards
· Complete all necessary documentation as required for maintenance of individual’s case management record as well as for accountability and billing purposes
· Provide the family with information, skills and supports related to enhancing the child’s development skills
· Plan home visits and be responsible for arranging provider visits
· Excellent computer skills, telephone skills, and leadership skills
Skills Required:
· Bachelors degree or higher in Human/Child Development, Social Work, or Social Services related field
· Reliable Transportation is mandatory and the ability to drive to multiple locations for home visits
· Case management experience highly preferred
· Excellent written and verbal communication skills
· Must be able to prioritize effectively and maintain efficient organization
· Valid drivers license, proof of valid car insurance, and able to pass a drug screen, background screening, and all required documentation for employment
Job Type: Full-time
Pay: $35,000.00 - $45,000.00 per year
