Description:
In collaboration with Genetic providers and team, provides coordination of care and patient education for patients in the inpatient and outpatient setting. The RN Care Coordinator will evaluate all aspects of care to ensure defined, consistent care is provided to patients at every level of disease, and will coordinate between subspecialty and primary care services and community resources.
Responsibilities
Job Requirement:
Responsible to the Division Director for providing complete interdisciplinary management of nursing services for individuals seen by the providers.
Participates and demonstrates ability to be an active member of a team-based group practice. Collaborates with the physician to develop a comprehensive care plan for patient.
Triage calls for patients (Collects and assesses patient data to determine patient needs and to promote optimal patient care); Document calls in Epic (when available for outpatients). Call in prescriptions to pharmacies as directed by providers.
Attend weekly clinics to provide nursing assistance to faculty and residents. Assist team members with patient examination and evaluation as necessary.
Assumes responsibility and accountability to ensure appropriate referrals are made for special procedures. This would include facilitating authorization for these procedures.
Assumes responsibility and accountability for tracking lab, pathology, cytology and radiology procedures ordered to ensure results are received and viewed by the responsible provider.
Arrange for appointments (new and follow-up) for patients referred to the service and patients who need follow-up after surgery or other treatment. This would include facilitating authorization for these appointments.
Make referrals to outside services as directed. Provide patient with educational brochures appropriate for the patient's treatment.
Serve as liaison between the care team and the patient, patient's family and as the patient's advocate as necessary
Prepare patient records and flow sheets for the team staffing. Provide current updates on each patient's treatment status to the team members as needed.

Experience Requirements
5 years-Minimum of three years nursing experience -required.
Ambulatory care setting - preferred
Education
High School Diploma or GED is required
Assocaites Degree is required
Bachelors - Nursing - Preferred
