About Job

Overview:

The Social Worker at Penn Asian Senior Services (PASSi) plays a vital role in supporting the health, well-being, and independence of older adults across the communities we serve. This position provides direct social services and case management, advocates for clients’ rights, and connects individuals and families with critical public benefits and community resources. The Social Worker will support seniors within PASSi’s community senior center, adult day program, and external community members, including seniors in the surrounding neighborhoods. The ideal candidate is compassionate, culturally competent, and committed to enhancing the lives of Asian and other underserved seniors through education, outreach, and client-centered care.

Position Duties and Responsibilities:

  • Provide direct social services and advocacy to PASSi clients, members, and local community residents.
  • Deliver case management services related to public health benefits (e.g., Medicaid, Medicare), government assistance programs (e.g., SNAP, LIHEAP), housing support (e.g., eviction prevention, senior housing applications), and other aging-related needs.
  • Offer information, referrals, and support to “drop-in” participants and ensure proper documentation and service delivery.
  • Build and maintain a strong referral network with external agencies; collaborate with community partners to improve service coordination.
  • Assist in the development and implementation of culturally and linguistically appropriate outreach strategies.
  • Represent PASSi at community events, health fairs, and partner meetings as needed.
  • Maintain accurate and timely documentation, including client charting, monthly reports, and case notes in accordance with agency standards.
  • Work collaboratively with internal teams and external partners to plan and participate in outreach events and special projects.
  • Assist in the planning and execution of workshops, public education sessions, and resource fairs that align with PASSi’s mission.
  • Perform other related duties and responsibilities as assigned.

Position Qualifications:

  • Bachelor’s degree in Social Work, Healthcare Administration, Public Health, Business Administration, or a related field required.
  • Minimum of two (2) years of experience working with older adults in a community-based or social service setting.
  • Experience in outreach, health education, or program coordination preferred.
  • Familiarity with the Asian communities of Philadelphia and surrounding regions is a plus.
  • Bilingual proficiency in English and Korean strongly preferred.
  • Strong interpersonal and communication skills; ability to work effectively with individuals from diverse backgrounds.
  • Proficiency in Microsoft Office Suite; experience with data entry and client management software is desirable.
  • Highly organized, detail-oriented, and capable of working independently and collaboratively.

  • Willingness to work occasional evenings and weekends; valid driver’s license and ability to travel locally required.
