CARE MANAGER, BSW – FAYETTE & COWETA COUNTY
Visiting Nurse Health System (Visiting Nurse) is hiring for a dynamic with a high EQ Care Manager, BSW, for our Members in Fayette and Coweta County. We seek a Social Worker, BSW, for our Service Option Using Resources in a Community (SOURCE) team who is a calm, compassionate, and empathetic presence, understanding and caring about our Members and meeting their needs. In this position you will perform...
- Maintains current knowledge of and arranges both SOURCE and non-SOURCE community-based services in collaboration with the Nurse Care Manager, the Member/caregiver.
- Monitors service delivery to each Member; follows-up on each service to determine if the service is appropriate and if it is effectively meeting the Member’s needs
- Develops appropriate Carepath specific to each Member’s condition and needs in consultation with the Member, Member’s caregiver, Nurse Care Manager, and service providers
- Identifies changes in Member condition/needs and adjusts the Carepath accordingly at every Carepath review Completes GDS, MMSE, falls assessment, and other advanced assessments as indicated including documentation and follow-up as determined by the advanced assessment score
- Documents all care management activity and service-related information, as required by DCH & VNHS policy or as directed by management, on a daily basis and ensuring a professional, non-bias and grammatically correct format
- Approves type and frequency of services appropriate to the Member’s needs while assuring that costs do not exceed the limitations established by the Division of Aging Services and the Department of Community Health.
- Complies with standards of promptness set forth by DCH and VNHS policy regarding specific activities:
Do you have a....
- Bachelor’s degree in human behavioral science which includes Development of Human Behavior, Child Development, Family Intervention Techniques, Diagnostic Measures or Therapeutic Techniques such as Social Work, Psychology
- Two years of experience in case management in a health care field
- Two years of experience with Medicare, Medicaid and other Funding Source
- Reliable transportation including a valid driver's license and proof of vehicle insurance
