We currently seek a full-time Care Manager to join our Workforce Development team! The Care Manager will improve discharge planning and re-entry outcomes for older incarcerated individuals by providing case management and overall supportive service in preparation for release. The Care Manager will assess each participant's specialized needs and connect them with appropriate services upon their release from New York City Jails. This position will require on-site presence at applicable facilities on the Department of Corrections Rikers Island. This position may be eligible for a hybrid schedule, however hybrid schedules are subject to change with or without notice.
Salary Range: $27.20 - $29.18 per hour

Requirements
Essential Duties:
- Perform screenings on each participant utilizing cognitive and geriatric assessments
- Create comprehensive transitional plans for participants
- Provide case management to participants who are soon to be released
- Utilize individual and group Interactive Journaling sessions to serve participants
- Travel to Rikers Island when needed to conduct in person or virtual sessions with participants
- Attend assigned training
- In-person attendance is required to collaborate with co-workers
- Perform other duties as assigned
- Associate’s degree in Human Services, Social Work or a related field with at least two years experience working in a social service setting supporting older adults; in lieu of a degree, High school diploma or equivalent with three years experience in a social service setting supporting older adults or working in a non-profit agency
- Experience providing cognitive and geriatric assessments preferred
- Prior case management experience
- Proficiency in Microsoft Office and Google Suite
- Competence in assessment and conflict-resolution
- Ability to provide strong attention to detail, time management and organizational skills
- Knowledge of the criminal justice system
- Ability to work in teams and collaborate effectively with people in different functions
- Ability to travel locally between Osborne Offices and DOC Facility located on Rikers
- Ability to take the initiative and drive for results
- Ability to communicate with all levels of an organization and build strong relationships
- Bilingual Spanish speaking individuals preferred.
