SOCIAL WORKER (CLINICAL) POSITION DESCRIPTION
The LCSW for Central Texas Helping Hands will work with various groups of individuals such as the elderly, people with mental health challenges, and people with developmental disabilities.
All employees are expected to provide the very highest level of service to CTHH clients and their families. In addition, their work ethic is expected to reflect the values and moral principles that inspired its founders.
Position Duties:
Serves as a social worker providing individual and group clinical counseling services using various evidenced based modalities as clinical indicated for adults to assist clients to identify and manage personal, psychological and interactive problems, associated with personal adjustment. Develops individualized treatment plans based on psychiatric diagnosis, identified problems and treatment needs for each case. Utilizes social work systems and psychological theories, principals and techniques in treatment plan development. Coordinates with patients social service agencies, primary care, specialty clinics and other organizations as needed to promote seamless services and support to patients. Provides limited case management to improve individual family and occupational functioning and mitigate crisis.
- Works with a variety of cases to include complex cases in which individuals are severely impacted by moderate to severe psychiatric diagnoses to include factors such as history of trauma, co-occurring conditions and suicide. Provides individual and group clinical counseling services using various evidence based modalities as clinically indicated for adults to assist clients to identify and manage personal, psychological and interactive problems, and to achieve effective personal, relational, and family development and adjustment: Collects bio-psycho-social information about patients using interview, case history, and observation techniques, and assessment methods. Analyzes information collected to determine psychiatric diagnosis, treatment plan and/or advisability of counseling or referral to other specialists or institutions. Reviews notes and information collected to identify problems and concerns. Consults reference material, for example the most current version of the Diagnostic and Statistical Manual for Mental Disorders (DSM), textbooks, manuals, and journals, to identify symptoms, make diagnoses, and develop therapeutic or treatment plan. Counsels clients, using counseling methods and procedures using approved evidence based treatment modalities to assist clients in gaining insight into personal and interactive problems, to define goals, and to plan action reflecting interests, abilities and needs. Evaluates results of counseling methods to determine the effectiveness of treatment used. Interacts with other professionals to discuss therapy or treatment, new resources or techniques.
- Provides minimal case management and resourcing services as needed to support the patient. Continuously assesses and responds to changing needs of patients based on environment. Provides concise documentation of care and recommendations. Provides reports as needed to management. Skill in written and oral communication especially in case formulation and presentation. Performs other duties as assigned.

Qualifications
Basic Requirements:
English Language Proficiency: Candidates must be proficient in spoken and written English.
Education: Applicant must have a master’s degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited.
Licensure: Persons hired to social work positions must be licensed or certified by the state of Texas Behavioral Health Executive Council Texas State Board of Social Worker Examiners to independently practice social work at the master’s degree level.
Preferred Experience:
Experience working in Medical Social Work
Experience in providing individual and group counseling
At Least 2 years of Medical Social Work experience
Experience in Case Management
Works well with others, using a team approach to meeting program and agency goals
Competently utilizes Microsoft Office (especially Words & Excel) and other software programs
Demonstrated Knowledge, Skills, and Abilities:
(a) Knowledge of community resources, how to make appropriate referrals to community and other agencies for services, and ability to coordinate services.
(b) Skill in independently conducting psychosocial assessments and treatment interventions to a wide variety of individuals from various socio-economic, cultural, ethnic, educational and other diversified backgrounds.
(c) Knowledge of medical and mental health diagnoses, disabilities and treatment procedures (i.e. acute, chronic and traumatic illnesses/injuries, common medications and their effects/side effects, and medical terminology) to formulate a treatment plan.
(d) Skill in independently implementing different treatment modalities in working with individuals, families, and groups who are experiencing a variety of psychiatric, medical, and social problems to achieve treatment goals.
(e) Ability to provide consultation and supervision services to new social workers, social work graduate students, and other staff about the psychosocial needs of patients and the impact of psychosocial problems on health care and compliance with treatment.
Physical Requirements:
The work requires moderate lifting (15-44 pounds), light lifting (under 15 pounds), light carrying (under 15 pounds), reaching above shoulder, use of fingers, walking (8 hours), standing (8 hours), hearing (aid permitted), and mental and emotional stability. Environmental factors include working alone and working closely with others.
