Key Responsibilities:

• Serve as the primary point of contact for clients and their families, addressing inquiries and concerns promptly.
• Conduct in-home assessments and develop individualized care plans
• Monitor client satisfaction and adjust care plans as needed
• Maintain accurate documentation and ensure compliance with company policies
• Monitor client satisfaction and implement improvements based on feedback.

Qualifications:

• Prior experience has Care Coordination (in-home care preferred)
• Strong organizational and communication skills
• Ability to multitask and remain calm under pressure
• Proficiency with Microsoft Office
• Valid driver’s license and reliable transportation

Requirements

• Previous experience in customer service or administrative roles preferred.
• Familiarity with medical terminology is a plus but not required.

• Ability to manage multiple tasks efficiently in a fast-paced environment.
