Key Responsibilities:

Provide direct support and case management for individuals in addiction recovery.
Conduct client assessments, develop care plans, and track progress.
Connect clients with essential services such as housing, employment, and healthcare.
Coordinate with medical professionals, counselors, and community resources.
Maintain accurate documentation and ensure compliance with regulatory guidelines.

Qualifications:Required:

Bachelor’s degree in Social Work, Human Services, Psychology, or a related field.
1+ years of experience supporting individuals with substance use, mental illness, or homelessness.
Strong problem-solving, organizational, and communication skills.
Flexibility to work variable shifts.

Preferred:

Master’s in Social Work (MSW) or Social Work Certification (LICSW, LSW, LCSW).
Experience using case management software and electronic medical records.
