Project PLASE (People Lacking Ample Shelter and Employment) was founded In 1974 to remedy the lack of housing and resources available to Baltimore’s homeless citizens. Our mission is to address homelessness by providing temporary and permanent housing and supportive services to homeless adults and families, particularly the most vulnerable and undeserved (i.e. people with mental illness, HIV/AIDS, addictions, ex-offenders, etc.) and by treating the whole person and empowering each individual to function at their highest possible level. We currently operate two temporary housing facilities and are in the process of converting an old elementary school in Irvington, Baltimore City into 34 Temporary Housing Units and 56 affordable permanent housing units. We have four Permanent Housing Programs. Across all programs we house and serve over 1,300 persons yearly.

We are currently seeking an HIV Counselor/Waitlist Coordinator for our Temporary Housing Facility in the Station North Arts District of Baltimore City. The successful candidate for this position will conduct outreach to locate homeless persons who need housing and services with an emphasis on locating and assisting individuals with HIV/AIDS, particularly with regards to health care access and utilization. In addition they will provide central intake for homeless persons seeking shelter, counseling clients who cannot immediately be placed at PLASE regarding alternative resources including medical care, support services, and other shelter accommodation.

ESSENTIAL FUNCTIONS

1. Conducts outreach to locate homeless persons who need housing and services with an emphasis on locating and assisting individuals with HIV/AIDS; Assists persons to access housing, medical care and other resources needed

2. Completes outreach encounter forms accurately and completely.

3. Provides information to homeless individuals seeking transitional housing who call or visit unannounced, including assuring they are able to schedule a formal face-to-face encounter to collect necessary information in order for the client to be placed on the waitlist (note, walk-in clients should be seen same day whenever possible.)

4. Conducts outreach to locate homeless persons with HIV/AIDS, especially those lost to medical care, in locations where homeless persons may congregate.

5. Accepts referrals from other human services agencies via telephone for their clients who are seeking transitional housing.

6. Refers appropriate clients who cannot be housed immediately in PLASE Transitional Housing to PLASE Permanent Housing staff for consideration of admission to PH case management programs (HOPWA, SHP), or MOHS Coordinated Access Application.

7. Assists clients who cannot be housed immediately with accessing needed resources and agencies in the community, including but not limited to health care, case management, and other transitional housing resources

8. Monitors medical appointments for all RW and RW eligible residents and applicants

9. Provides follow up and case management for a small case load of individuals with HIV/AIDS, as assigned

10. Assists with obtaining and evaluating feedback from current and former clients for input, program improvement, etc.

11. Updates paper and electronic client records as required.

12. Maintains other records, as required by agency and funders.

13. Conducts in-take for all new RW housing clients, including completion of Central Client Intake, and HIV intake, and CD4 & VL tracking.

14. Ensures RW eligibility documentation are presented to the facility for each new RW client completing in-take.

15. Monitors all transitional housing in-takes to ensure all in-take electronic records are completed within established time-lines; Takes appropriate action to notify staff and managers when time-lines are not met.

16. Monitors HIV health care progress for all HIV positive clients (not just RW clients), by running and analyzing MIS reports on a regular basis (weekly, monthly, quarterly, annually, and as requested).

17. Reports analysis of HIV health care progress as requested, in both verbal and written form.

18. Maintains daily database of clients housed in transitional shelter, monitoring bed utilization so as to limit the number of empty beds on any given day.

19. Receives applications for PLASE Permanent Housing applications and responds according to policy.

20. Tracks appointments and attendance for all RW clients.

21. Monitors client medical appointments and possibly medication toward improved health with HIV

22. Assures HIV testing and counseling, for those who do not know their status.

23. Maintains database of permanent applicants and waitlists.

24. Conducts analysis of permanent housing databases and waitlists as requested and reports verbally and in written form.

25. Liaisons with social service agencies on behalf of PLASE;

26. Works collaboratively as part of team effort on unit;

27. Maintains database of clients seeking shelter, those already housed and bed nights occupied per day.

28. Communicates with Mangers of Transitional Housing about filling empty beds or other relevant issues;

29. Participate in strategic planning meeting and implement Quality Improvement Plan (QIP

30. Provides case management and/or referral, counseling, and support information to homeless persons who have mental health problems, developmental disabilities, substance abuse issues, have AIDS or are HIV+ or are at risk for AIDS, etc.;

31. Acts as liaison between the client, Project PLASE and relevant community-

based organizations, services, health facilities, etc.

32. Prepares information for reports to funding agencies as required by supervisors

33. Inputs all client data and information into client databases as required.

34. Seeks out and incorporates into work response one activity annually to address the larger causes of homelessness and poverty;

35. Performs other duties as assigned.

REQUIRED KNOWLEDGE, SKILLS/ABILITIES

1. Ability to gain thorough knowledge of Project PLASE’s policies and procedures;

2. Ability to act as a representative of Project PLASE to the public;

3. Ability to effectively communicate and empathize with individuals from diverse backgrounds especially those with special needs;

4. Ability to create and maintain effective working relationships with other professionals;

5. Ability to maintain accurate written and computer based records;

6. Ability to run electronic reports from electronic management information systems;

7. Ability to format complex excel spreadsheets in order to analyze key data;

8. Ability to express data analysis in both written and verbal form; and

9. Ability to use sound judgment and remain calm in crisis or emergency situations.

10. Strong organizational and planning skills, and ability to prioritize the work duties

11. Superior interpersonal skills including dealing adeptly with all types of personalities

12. Ability to work independently, and make independent decisions

13. Proficiency in databases is required; Proficiency is preferred in HMIS, Careware, and Apricot Bonterra databases, including running reports, or the ability and desire to learn these three databases;

14. Proficiency in MS Office to include MS Excel and MS Word

15. Ability to touch type

EDUCATION AND EXPERIENCE

1. Bachelor’s Degree in a Human Services related field

2. Two or more years related experience;

3. Experience counseling clients with HIV disease and addictions

· Individuals with a Bachelor’s in Social Work are encouraged to apply.

CERTIFICATIONS AND LICENSES

· Valid Maryland driver’s license and evidence of insurability

PHYSICAL AND ENVIRONMENT CONDITIONS

Work requires light physical effort in the handling of light materials or boxes in non-strenuous work positions of up to 15 pounds and/or continual standing or walking 60+% of the time, and may include sitting at a computer for extended periods.

Work will include going into the community to conduct outreach to persons who are homeless.

Work environment involves everyday risks or discomforts which require normal safety precautions typical of such places as offices, meeting and training rooms, residences, and vehicles, as well as city streets, e.g. use of safe work place practices, avoidance of trips and falls, observance of fire regulations and safely operating a motor vehicle.

May be exposed to aggressive human behavior.
