About Job

Community-Based Adult Services/Adult Day Health Care facility looking for an MSW. Experience in ADHC is a PLUS.

Masters Social Worker:
- Masters in Social Work; OR
- Masters in psychology, gerontology or counseling with ONE year of experience providing social services; OR,
- Bachelor's degree in social work with TWO years of experience providing social services.

MSW Assistant Job Duties:
- Provide assessment and reassessment
- Development and implementation of Individual Plan of Care
- Individual and group counseling to elderly participants as per each individual plan of care
- Planning and coordination of discharge plan
- Documentation of provided services
- Provide various care management services to participants in order to obtain needed services such as IHSS and Access
- Assist with Primary Care Physician appointments
- Provide case management to participants in need of housing referrals and access to social welfare programs
- Complete all relevant paperwork and maintain participants' files

* MSW starting salary at $25 per hour.
