Overview
We are seeking a dedicated and detail-oriented Intake Coordinator to join our dynamic team. The ideal candidate will play a crucial role in managing client intake processes, ensuring a smooth and efficient experience. This position requires strong organizational skills, proficiency in home care/health care management, and familiarity with EHR software systems.

Duties

• Collect and verify patient information
• Input data accurately into the electronic health record system.
• Assist with timely start of care
• Maintain organized patient files and ensure compliance with privacy regulations.
• Collaborate with branch staff to ensure seamless communication regarding client needs.
• Assist with clerical tasks such as filing, scanning, and managing correspondence.
• Utilize software systems such as Well Sky, Empeon, Apploi, etc.

Requirements

• Previous experience in home care preferred.
• Strong clerical skills with attention to detail and accuracy.
• Excellent communication skills, both verbal and written.
• Ability to work collaboratively in a fast-paced environment.
• A commitment to providing exceptional patient care and support.
