Overview
We are seeking a dedicated and organized Care Coordinator to join our team. The ideal candidate will play a crucial role in ensuring that patients receive the highest level of care and support throughout their healthcare journey. This position requires strong communication skills, attention to detail, and a solid understanding of medical terminology. The Care Coordinator will work closely with healthcare providers, patients, and their families to facilitate effective care management.

Duties

  • Respond to inquiries from prospective clients and their families, warmly introducing them to our services.
  • Schedule initial in-home assessments and assist in gathering essential client information, understanding their unique needs.
  • Work with our team to help craft personalized care plans focusing on daily living activities, companionship, and household support.
  • Create, manage, and optimize caregiver schedules, ensuring all client shifts are consistently covered.
  • Proactively address call-outs and emergencies, finding immediate replacements to maintain continuous care.
  • Verify caregiver EVV for accuracy, ensuring seamless payroll and billing processes.
  • Serve as the go-to person for clients, families, and caregivers for all scheduling inquiries, concerns, and updates.
  • Communicate care plan changes and updates clearly to caregivers, ensuring everyone is on the same page.
  • Keep all client and caregiver files organized and confidential.
  • Participate in an on-call rotation to handle after-hours needs and emergencies.

Qualifications

  • High school diploma or GED required; Associate's or Bachelor's degree in a related field (e.g., healthcare administration, social work) preferred.
  • Proven experience in scheduling, coordination, or customer service, ideally within a home care or healthcare setting.
  • Exceptional verbal and written communication skills – you're a clear and compassionate communicator!
  • Superior organizational abilities and time management skills; you can juggle multiple tasks with ease.
  • A natural problem-solver with a proactive approach.
  • Proficiency with office software (Microsoft Office Suite) and experience with home care scheduling software is a big plus.
  • A genuinely empathetic and compassionate nature, with a true desire to make a difference in people's lives.
  • A collaborative team player who thrives in a supportive environment.
  • Adaptable and flexible to meet evolving client needs and agency priorities.

Job Types: Full-time, Part-time

Pay: $22.50 - $25.00 per hour

Expected hours: 40 per week
