Job Title: Lead Care Manager, Children and Families
Department: Housing Justice
Pay Rate: Hourly - $33.65-$36.06
Position Summary
As part of Community Action Marin’s launch of CalAIM Enhanced Care Management (ECM), the Lead Care Manager, Children and Families (LCM-CF) will provide high-touch, person-centered care coordination to children and their families who are currently on the agency's childcare services waitlist and meet ECM eligibility requirements. The LCM-CF builds trusted relationships that support parents and legal guardians as they identify and meet goals for addressing the social drivers of health and the physical, behavioral, and oral health needs of their children and families. Operating in accordance with CA Department of Education, Head Start Performance Standards and the CA Department of HealthCare Services (DHCS) Medi-Cal Transformation program, the LCM-CF will manage a caseload of 20 families, coordinating health care benefits, providing education, and facilitating client access to a range of health and social care in a timely and dignity centering manner.
Primary Responsibilities
- Care Management & Coordination
- Conduct comprehensive assessments and develop individualized care plans for children and families enrolled in ECM.
- Coordinate services across medical, behavioral health, education, housing, and social service systems to ensure timely access to care.
- Meet regularly with parents and caregivers where they are (in-person whenever possible) to monitor progress, address barriers, and adjust care plans as needed.
- Family Engagement & Advocacy
- Build trusted, culturally responsive relationships with children and their families grounded in the Whole Family Approach and supporting self-determination and long-term well-being.
- Serve as a strong advocate for family needs across systems and support navigation of complex bureaucracies.
- Program Leadership & Team Support
- Collaborate closely with the CalAIM team and other CAM programs (e.g., Children and Family Services, Economic Justice) to ensure seamless connection to services.
- Support the implementation and refinement of ECM workflows and documentation systems related to children and family care.
- Documentation & Compliance
- Maintain accurate and timely case notes, assessments, and data entry in accordance with ECM and agency protocols.
- Ensure services meet Head Start, DHCS, and CalAIM ECM requirements, including proper care coordination documentation and reporting.
Required:
- Bachelor’s degree in Social Work, Public Health, Child Development, Human Services, or a related field.
- Minimum of 2 years of experience in care coordination, case management, or family support services, preferably with Medi-Cal populations ages 0-5.
- Demonstrated experience working with low-income, diverse, and/or underserved communities.
- Strong understanding of child and family development, trauma-informed care, and systems navigation.
- Excellent communication, documentation, and organizational skills.
- Bilingual (Spanish/English) required.
Preferred:
- Knowledge of CalAIM, ECM, and managed care systems.
- Familiarity with childcare subsidy programs, waitlist management, and early childhood systems in California.
