Pilsen Wellness Center, Inc. is a community-based not-for-profit agency that provides culturally sensitive services to a multicultural population with limited financial resources. The organizations mission has remained providing quality human services to economically disadvantaged children and families. The agency is comprised of five divisions - Mental Health, Substance Abuse, Youth Services, Early Intervention and Education which are currently supported through 17 different locations.
POSITION SUMMARY

We are seeking a Full-Time Mental Health Professional to join our HOGAR Program located at 2938 E. 89th St. Chicago, IL 60617

WORK SCHEDULE
The work schedule available is: Monday - Friday 9:00am - 5:00pm

DUTIES AND RESPONSIBLITIES

This individual is to assist the HOGAR participants to transition from long-term care facilities back in the community by helping them find appropriate independent living arrangements that will support their recovery and success. This individual will also provide ongoing support and expertise through comprehensive assessment, planning, implementation and overall evaluation of individual participant needs.

EDUCATION and/or EXPERIENCE

This individual must have a Bachelor Degree in Psychology or a related field OR 5 years’ experience in the Mental Health Field. Familiarity with Medicaid Rule 132 desired. Valid Driver's License required. Bilingual (Spanish) required.
