Position Overview:
We are seeking a highly motivated and compassionate Care Manager to join our team in Edmond, OK. This full-time role will support patients enrolled in CCM and PCM programs—spanning primary care and a variety of specialties including psychiatry, neurology, pain management, cardiology, endocrinology, and more. The ideal candidate is an experienced caregiver or clinical support professional with a strong background in patient communication and care coordination.
This position includes direct oversight of an administrative support team and reports to Una Health’s in-house pharmacist, who provides ongoing clinical guidance and support.
Key Responsibilities:
• Conduct regular, structured outreach calls to patients enrolled in Chronic Care Management and Principal Care Management programs.
• Monitor and document patients’ conditions, goals, and barriers to care using our care coordination platform.
• Support medication management, appointment adherence, and provider communication.
• Coordinate services across a range of medical specialties and primary care providers.
• Supervise and support an internal team of administrative staff to ensure timely care follow-up.
• Collaborate closely with Una Health’s pharmacist for clinical input and patient care planning.
• Follow best practices for care documentation, escalation, and triage.
Preferred Qualifications:
• Certified Medical Assistant (CMA), Certified Nursing Assistant (CNA), or Home Health Aide (HHA) preferred.
• Minimum of 5 years of experience in home health or clinical office settings.
• Strong interpersonal skills with a passion for patient engagement and advocacy.
• Proficiency using healthcare software or patient documentation platforms.
• Experience in chronic disease management, behavioral health, or specialty care is a plus.
