About Job
- Identify and evaluate social and/or physical factors which may either impede full recovery from illness/surgery or prevent the patient from maintaining a state of well-being
- Communicate these factors to the healthcare team and performs interventions, as indicated.
- Assists patient and their families with procurement of community resources, as needed
- Assist with the overall discharge planning process with the patient and interdisciplinary team.
- Education: Must be a graduate of an accredited social work program
- Licensure and Certifications: Licensed Social Worker (LSW), required.
- Experience: Previous experience with social work standards of practice and ethics, required. Previous experience in an acute care setting, required.
