Job Title: CPAP Patient Care Coordinator

Location: Hudson, WI

Position Overview

Hudson Sleep & CPAP has been helping western Wisconsin patients with their sleep health since 2019. We offer home sleep testing as well as CPAP therapy. We are located in the beautiful new Hudson Medical Center and are seeking a proven health care professional to join our team and provide best in class services.

This position primarily conducts DME patient follow-up. The position is responsible for contacting patients to encourage set up of CPAP equipment, order completion, patient scheduling with our CPAP Specialist, entering patient records in appropriate data bases, other documentation and distribution and collection of HST devices This position also supports Insurance verification needs.

Duties and Responsibilities

  • Receive and process all incoming DME referrals.
  • Outbound & Inbound phone calls
  • Send and receive patient records
  • Complete and/or update Microsoft excel related tracking reports
  • Create and maintain patient charts/templates as appropriate.
  • Obtain all necessary documents required for scheduling CPAP or Sleep testing
  • Verify patient insurance coverage.
  • Collecting payment
  • Obtain all necessary prior authorizations for CPAP and Sleep testing
  • Assist with patients’ questions, make appointments to see a Clinical Specialist if warranted.
  • Back up support for patient’s CPAP needs including supply request, downloads, mask exchanges, etc.
  • Distribute and collect HST devices to/from patients
  • Perform other general duties that may be assigned from time to time.

  • Outstanding patient/customer service skills
  • Good business judgement with excellent organization and communication skills
  • Ability to work with minimal direction, but understands the value of interaction with co- workers, patients, and providers
  • Excellent computer skills and ability to quickly learn new data base programs
  • Professional demeanor
  • Desire to drive results with the highest level of patient care and professionalism
  • Familiarity with CPAP therapy and equipment helpful by not required

Qualifications

  • Minimum of 1-year healthcare experience
  • EMR experience Preferred but not required
  • Brightree experience helpful but not required

Job Type: Full-time

Pay: $20.00 - $23.00 per hour
