About Job

As Case Manager provides evaluation, consultation and intervention with children and youth, and their families, in the Gang Intervention Reduction Program (GRIP). Also participates in conducting psychosocial programming with this population as part of a clinical team composed of Juvenile Probation and Mental Health staff in an effort to reduce juvenile gang involvement.EXAMPLES OF WORK PERFORMED:
  • Works with youth and their families in mental health, home or office settings.
  • Provides case management and some skills training with children and families.
  • Makes assessment and psychosocial evaluations and recommendations.
  • Refers families of clients to appropriate social welfare and health agencies to obtain assistance.
  • Performs Related Work as Required.
Experience, Training, and Education:
  • One year of case management preferred.
  • Must have current valid driver’s license and current and continued proof of liability insurance.
  • Bachelor’s Degree (CWI) or Master’s Degree (CW II) in Psychology, Social Work or related field
KNOWLEDGE, SKILLS AND ABILITIES:
  • Knowledge of emotional problems and treatment approaches.
  • Ability to establish and maintain positive working relationships with consumers and colleagues.
  • Good work habits and organizational skills.
  • Familiarity with computer operation.
  • Level of general health permitting stooping, bending, climbing stairs and participating in moderately active recreational activities.
  • Physical ability to perform CPR and PMAB skills.
  • Must have and maintain a level of general health which permits the performance of job duties.
  • Bilingual Preferred
  • Ability to lift up to 30lbs.
  • Ability to travel locally 50% and regionally 5% and to transport clients.
  • Skills Training, Linking/Monitoring Internal/External Services and reduce recidivism
  • Must have excellent clinical documentation skills.
