In 2022, Bluestone Accountable Care Organization (ACO) was the best performing ACO in the country as measured by the overall savings per Medicare beneficiary.Position Overview:The Behavioral Health Care Manager / Dementia Case Manager (Part-time 24 hrs./week) is a core member of the collaborative care team, which includes a Bluestone's Primary Care Providers, Nurses, Care Managers and other medical professionals. The Behavioral Health Care Manager is a patient-focused role, primarily working with a complex geriatric population and their families within their home environment (Assisted Living, Memory Care, and/or Independent Living). Additionally, this role is an expert in the field of dementia care, Alzheimer's, memory loss, and/or mental health targeting interventions that improve the patient's overall quality of life. Schedule: Part-time position, day shift hours, no evenings, weekends or holidays.Location: This position entails a mix of remote work, as well as direct patient care mainly throughout Apopka, Celebration, Mount Dora, Ocoee, Orlando, Tavares, Winter Garden.Salary Range: $33,000 - $39,000, Salary will be commensurate with experience.Qualifications:Education/Certification/ExperienceFormal education or specialized training in behavioral health, including social work, nursing, psychology, gerontology, music therapy or related fieldsOne or more years of experience in memory care and/or dementia-related careValid driver's license requiredResponsibilities:Provide ongoing necessary education and support to patient's care team on Alzheimer's disease and related memory loss/dementia and their impact on cognitive functionProvide ongoing necessary education and support to patient's care team on Mental Illness diagnoses and their impact on cognitive functionEstablish care plans that outline interventions to reduce behavioral episodes and improve function and safetyProvide behavioral interventions using evidence-based techniques such as motivational interviewing, problem-solving, modeling, active listening, other techniques as appropriateIdentify and provide de-escalation strategies and crisis resources for caregivers, patients, and families Provide effective non-pharmacological behavior prevention and reduction solutionsIdentify strategies to anticipate and calmly de-escalate distress behaviorsSystematically track treatment response and monitor patients for changes in clinical symptoms and treatment side effects or complicationsComplete validated rating scales monthly to monitor and assess response to care plan interventionsParticipate in weekly caseload consultations with psychiatric consultantsFacilitate referrals for the clinically indicated services outside of the organization (e.g. mental health specialty care, social services, support groups, etc.)Act as a Bluestone ambassador for community staff through education and relationship buildingQualifications Continued:Knowledge/Skills/AbilitiesKnowledge of behavioral health, dementia, and care planningKnowledge of assessments, screenings, and care planning for mental health disordersAbility to engage patients in a therapeutic relationship when appropriateAbility to work independently with excellent time-management and organizational skillsAbility to maintain professional relationships with patients and other members of the care teamAbility to communicate effectively and professionally, both verbally and in writing, with diverse populationsIntermediate-level computer proficiency with email, fax, word processing, spreadsheets, and databasesExcellent customer service skillsDemonstrated ability to read, write, speak, and understand the English languageBluestone Benefits:Part-time hours PTO accrualMileage reimbursement program for field employees Company sponsored cell phone, laptop and scrubsWorking within regular business hours
