The Behavioral Health Care Manager is a patient-focused role, primarily working with a complex geriatric population and their families within their home environment (Assisted Living, Memory Care, and/or Independent Living). Additionally, this role is an expert in the field of dementia care, Alzheimer's, memory loss, and/or mental health targeting interventions that improve the patient's overall quality of life.
