Are you passionate about making a difference in the lives of individuals with developmental disabilities? Do you have experience in roles such as HCBS Coordinator, Care Manager, Group Home Manager, or Direct Support Professional? Endeavor HCBS is looking for a dedicated Care Coordinator fluent in Spanish to serve a caseload of primarily Spanish-speaking clients and families in our East Valley region.

What You'll Do:

• Visit members' homes to build meaningful relationships and assess needs

• Implement and monitor Person-Centered Service Plans (PCSPs)

• Participate in PCSP meetings to advocate for positive changes

• Interview and match Direct Support Professionals to client needs

• Support and mediate the relationship between provider, family, and the care team

• Educate Spanish-speaking families about HCBS services and resources

• Attend team meetings and company events to stay engaged and informed

• Remain compliant with DDDAHCCCS requirements and policies

Requirements:

• Fluency in Spanish and English (both verbal and written)

• Prior experience in HCBS, care coordination, direct support, or group home management

• Strong understanding of Arizona's DD system and services

• Proficiency with Google Drive and Apple products

• Ability to travel locally for member visits (mileage reimbursed)

• Valid Level One Fingerprint Clearance Card

• Valid driver's license and reliable personal transportation

• Must pass APS and CPS Registry background checks

• Passion for serving the special needs community

Location: Mesa Office Higley RdBaseline Rd (travel required for home visits)
Schedule: Monday-Friday, full-time
