Small business looking for social worker to assist with complete nursing facility visits & relocating elderly and disabled individuals from nursing facilities to the community in the Nueces County area and surrounding counties. Position requires a degree and experience with person centered planning, developing individualized care plans, conducting outreach and education, working in a fast paced environment and adhering to guidelines and meeting timeframes. Must be able to think critically, problem solve, work independently and document details of interactions with clients.

As a Relocation Specialist, you will be instrumental in guiding clients through the relocation process, ensuring a seamless transition to their new homes. Reporting to the relocation manager, you will leverage your core skills in customer service and communication to address client needs effectively.
