Job Summary:
The Community Liaison:

  • Contacts, presents and develops referral relationships.
  • Works with intake to insure growth goals are met.
  • Meets with Administrator weekly to discuss successes and opportunities.
  • Revisits Marketing/Networking/Business Plan as required.

Essential Functions:

  • The ADMISSIONS COORDINATOR adheres to legal and ethical requirements regarding the products and services of the agency.
  • The ADMISSIONS COORDINATOR generates referral stream by creating and maintaining relationships with referral and medical facility sources.
  • The ADMISSIONS COORDINATOR makes prospect calls to potential new referral sources; implements contracts and agreements to insure that sales goals are met.
  • The ADMISSIONS COORDINATOR develops and maintains prospects, clients, and referral sources database for contact and mailing lists.
  • The ADMISSIONS COORDINATOR participates in marketing and case management meetings with agency team.
  • The ADMISSIONS COORDINATOR tracks networking efforts and reviews with Administrator on a weekly basis.
  • The ADMISSIONS COORDINATOR prepares accurate documentation regarding contacts, timesheets, and mileage.
  • The ADMISSIONS COORDINATOR adheres to the marketing budget and allocates resources appropriately.
  • The ADMISSIONS COORDINATOR monitors referral trends with appropriate follow-up.
  • The ADMISSIONS COORDINATOR participates in community events that promote the agency.
  • The ADMISSIONS COORDINATOR promotes a positive self-image and agency. Conducts self in a professional manner both verbally and nonverbally.
  • The ADMISSIONS COORDINATOR communicates effectively with business office and clinical personnel regarding potential referrals or issues with clients.
  • The ADMISSIONS COORDINATOR develops weekly marketing strategy and adheres to plan.
  • The ADMISSIONS COORDINATOR acts as a resource for clients, caregivers, and family members.
  • The ADMISSIONS COORDINATOR informs Administrator or CEO of issues needing further investigation.
  • The ADMISSIONS COORDINATOR, if requested, meets with client and family members to discuss company services.
  • The ADMISSIONS COORDINATOR performs other duties as assigned or as needed.

Education:

  • Degree from accredited university in Health Care or Marketing Management or related experience

Training Requirements:

  • A minimum of two years marketing experience in home health services preferred.
  • Requires proficiency in contact management software (Office, Excel, Power Point)

Knowledge Requirements:

  • Must be able to talk through oral and written expression to convey information effectively.
  • Must be able to read, write, speak and understand the English language.
  • Have the ability to speak clearly and present well in a group.
  • Able to develop rapport with client management and maintain strong working relationships.
  • Hardworking, self-starter that is committed to meeting growth goals.
  • Willing to set and achieve challenging growth goals and successfully manage accounts.
  • Demonstrate excellent organizational skills.
  • Ability to manage own time.

Professional Requirements:

  • Maintains and ensures HIPAA compliance.
  • Responsible and accountable to the Administrator and the CEO regarding time availability and performance.
  • Maintains good rapport and cooperative working relationships with patients, physicians, referrals sources, regulatory agencies and staff.
  • Adheres to company dress code.
  • Attends annual review and departmental in-service training.

  • The applicant does not have record of a crime, offense or misdemeanor as defined in 97.241 of the Texas Administrative Code, Title 40, and Chapter 97.
  • The candidate must adhere to high personal and professional ethical values.

Job Type: Full-time

Pay: $65,000.00 - $85,000.00 per year
