Job DetailsJob Location: Richmond, VAPosition Type: Per DiemSalary Range: UndisclosedTravel Percentage: Road WarriorDescriptionPer Diem opportunity for an LMSW - Social Worker in the Richmond area!Constellation Hospice is looking for a compassionate and empathetic Social Worker (LMSW) to work with our hospice patients. This role will involve providing care in the patient's homes and at Skilled Nursing Facilities.Responsibilities:

Provide Clinical Social Work to our hospice population. Carries out social evaluations, including family dynamics, caregiver abilities, communication patterns, neglect or abuse and plans intervention based on evaluation findings
Educate patients and caregivers on disease process, medications, plan of care and individualized treatment plan
Maintains and processes all documentation in compliance with agency standards utilizing the electronic medical record-keeping system. Submits all clinical documentation, time records and other information in the required time frame

QualificationsQualifications:A graduate of a master’s program in social work (MSW) accredited by the Council on Social Work EducationORA graduate of a bachelor’s program in social work accredited by the Council on Social Work Education. Meets personnel qualifications stated in §418.114(b)(3).The employee has the required licensure, education, training, and qualifications required by rule, regulation, law and/or statute to qualify for the role in their state. Previous home care/hospice is required. Experience in a hospice care preferred.Demonstrates good verbal and written communication, and organization skills.Possesses and maintains current CPR Certification (per state regulations)Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order.
