About Job

Position Overview
Facilitates patient care and patient transitions through the health care system while managing the timely and appropriate use of resources. Collaboratively assesses, plans, coordinates, monitors and evaluates patient treatment plans to promote effective utilization of services throughout the continuum of care and to ensure quality outcomes. Monitors the patient care process and collects data to evaluate and improve care and collaborates with physicians and other health care workers to provide comprehensive care including health risk appraisal, health promotion and clinical outcome studies. Educators' payers, providers and the public and serves as the information source regarding state and national health care trends; initiatives, which enhance the provision of, care and ensure efficient resource utilization.
Primary Department or Work Location:
Care Management, UHS Wilson Medical Center
Work Shift and Schedule:
This is a per diem position, which means you will work on an as needed, agreed upon basis. Working hours will be assigned by your manager.
Compensation Range:
$40.70 - $61.05 per hour, depending on experience
This position is not eligible for benefits.

Education/Experience
Minimum Required:
  • Associate in applied science in Nursing with a minimum of five years of experience in acute/tertiary facility.
Preferred:
  • Bachelor’s of Science in Nursing with 3 years of experience in acute/tertiary facility.

License/Certification
Minimum Required:
  • NYS Registered Nurse
