Overview

As a Discharge Planner, the primary responsibility involves coordinating and executing discharge planning services, identifying local resources, and arranging appointments with programs or providers to ensure a smooth transition for individuals. Additionally, Discharge Planners may offer individual and/or group psychoeducational sessions for individuals scheduled for release into the community, fostering their preparedness and support network. Through careful coordination and psychoeducational efforts, the Discharge Planner plays a pivotal role in facilitating successful transitions for individuals re-entering the community.

Responsibilities

  • Provide discharge planning services, adhering to contract requirements, accreditation standards, and organizational policies, covering medication-assisted treatment (MAT), medical, and mental health programs.
  • Conduct individual and/or group psychoeducational sessions for individuals scheduled for release, fostering preparedness for their transition into the community.
  • Establish and maintain relationships with community agencies serving individuals with mental illness or serious medical conditions, facilitating applications for financial assistance and resumption of benefits.
  • Coordinate medication and medical needs for individuals upon release, including potential residential placement, and assist in making appointments and referrals to mental health and medical healthcare agencies/providers.
  • Document services in health records, including community provider contacts, and collaborate with Facility Staff, Institutional Parole Officer, and relevant staff while adhering to security regulations and confidentiality requirements.

Qualifications

Education

  • Bachelor’s degree in social work, psychology, or related field from an accredited college or university is .
  • High school diploma with experience will be considered

Experience

  • Previous experience in developing successful transition plans involving community resources for persons with medical, substance use, and/or mental illness is .

Licenses/Certifications

  • Must maintain CPR certification, and any other certifications (such as First Aid) required by contract.
