As an Admissions Care Coordinator III, you will support the Access Center operation efforts in a
variety of initiatives for Pinnacle Treatment Centers. You are responsible for timely
response to Access Center inquiries to facilitate and coordinate admission of those seeking
treatment at a PTC facility.

Requirements:
- High School diploma or equivalent
- Proficiency in Microsoft Office Applications, EMR/EHR systems, and Salesforce
- One (1) year of experience in substance abuse or mental health field
- Tenure of at least 12 months.
Preferred
- Bachelor’s Degree in a related field.
- Call Center experience
- Experience working in a team setting
Responsibilities:
- Receive and process inbound inquiries from phone, email, chat, omni channel and faxes.
- Assess potential patients treatment needs, locating facility and level of care that coincide with patient’s clinical and medical needs for both residential and outpatient (all states).
- Document patient assessment accurately and promptly into a system of record, ensuring all pertinent information is inputted in a quick and efficient manner.
- Determine and disseminate patient’s insurance eligibility and benefits, assessing viable options for treatment within Pinnacle Treatment Centers.
- Communicate all relevant information to the supervisory team regarding patients and the facility.
- Attend team meetings and complete all training courses as required.
- Assist Access Center Leadership with day-to-day tasks and special projects.
- Provide support in the absence of the Care Coordinator Supervisor.
- Complete all essential functions of Care Coordinator I and II, as needed.
- Other duties as assigned.
