Synergy Lifestyle Center is an outpatient Intensive Cardiac Rehab (ICR) center implementing Dean Ornish's Undo It program. We encourage lifestyle (Exercise, Stress Management, Community, and Nutrition) as a way to not only treat but REVERSE heart disease, and as the research shows, potentially helping to reverse other chronic diseases as well. Our program consists of 9 consecutive weeks where our participants attend 2 days a week for 4 hours per day. Each of the 4 hours focuses on 1 of the core elements in the following order: Exercise, Stress Management, Community via Group Support, and Nutrition. The goal is to provide education, in addition to an experiential component. Each patient participates in an hour of exercise with our exercise physiologists, guided stress management with our certified yoga instructor, group support with our licensed mental health counselor, and also receives a meal focused on whole-food, plant-based, and low-fat nutrition guidelines along with an hour-long education lecture taught by our registered dietitian. We are open Mon-Thurs and have our "cohorts" (a cohort is a group of participants in the program that will attend the full 9 weeks together) running on either Monday and Wednesday or Tuesdays and Thursdays.

Responsibilities:
- Facilitates and delivers the group support portion of the Program as trained per guidelines and protocol.
- Provides support to participants in developing the skills necessary for integrating lifestyle changes into daily life.
- Monitors group support adherence for all participants and provides additional explanation of guidelines on an individual basis as needed.
- Facilitates the Program delivery team’s transdisciplinary development process.
- Attends and participates in weekly team meetings/case conferences.
-Participates in the development of the interdisciplinary treatment plan for each participant.
-Participates in the monitoring, documentation, and communication of the participant’s response to the interdisciplinary treatment plan throughout the Program
- Assists the Program Director with efforts to actively promote the Program to prospective participants and healthcare professional as needed
-Attends and participates in mandatory monthly element specialist calls hosted by the Ornish National Training Center
-Assists the Program with efforts to actively promote the Program to prospective participants and health care professionals as needed
-Personally supports the programs principles, philosophy, and practices
-Complies with HIPAA privacy and patient confidentiality policies and procedure
