Job Description:

Grand Villa of Port Charlotte is seeking a dedicated and experienced Memory Care Manager to join our team at our Assisted Living Facility. The ideal candidate will have a strong background in memory care, particularly with dementia, and will be committed to providing exceptional care to our residents.

Key Responsibilities:

  • Assist in overseeing the memory care program, ensuring high-quality care for residents with dementia and other memory-related conditions.
  • Develop and implement individualized care plans in collaboration with residents, families, and the care team.
  • Train and supervise staff in best practices for memory care and support.
  • Monitor and evaluate the effectiveness of care plans and make adjustments as necessary.
  • Foster a supportive and engaging environment for residents, promoting their dignity and independence.

Qualifications:

  • Degree as a Licensed Practical Nurse (LPN) is required.
  • Proven experience in memory care, particularly with dementia.
  • Strong leadership and communication skills.
  • Ability to work collaboratively with a diverse team.
  • Compassionate and patient-centered approach to care.
