Join our Recuperative Care Program (RCP) team and make a direct impact on the lives of individuals experiencing homelessness after hospital discharge. As an RCP Case Manager, you’ll serve as the primary liaison for healthcare facilities, assess placement needs, and coordinate care in partnership with our RN and Homeless Services leadership. You’ll guide clients through intake, develop personalized care plans, and support their journey toward physical stability and long-term housing solutions. Collaborate with local agencies to ensure continuity of care and help clients transition into more permanent settings.
Qualifications:
- Bachelor’s Degree in Social Work, Psychology, or related field, OR
- 3 years of related work experience.
- Any combination of qualifying education and experiences may be acceptable.
- Experience in medically related direct client services including assessment, evaluation, treatment/discharge planning, referral and follow-up to a population comprised of individuals experiencing homelessness, parolees or probationers, individuals experiencing mental health challenges, the chronically ill, or high-risk youth is preferred but not required.
- Interface with healthcare facilities to address admission requests and ensure appropriate placement by meeting with both healthcare staff and the client prior to admission.
- Assess client eligibility for RCP services based on current physical needs and ability to maintain appropriate levels of Activities of Daily Living.
- Collaborate with the RN and RCP staff to complete program assessments and develop care plans with short- and, when appropriate, long-term goals and the steps to achieve them.
- Maintain a working knowledge of issues related to chronic homelessness and medical conditions, including PTSD, traumatic brain injury, substance use disorders, and other mental health challenges.
- Work collaboratively with the County Behavioral Health Therapist to coordinate therapy referrals as needed.
- Ensure timely follow-through for all necessary medical, mental health, and substance use appointments by working closely with clients and providers.
- Advocate for clients to reduce barriers to accessing essential services.
- Maintain professional relationships with clients and conduct all activities in accordance with program policies and case management standards.
- Monitor client behavior for compliance with program expectations and provide consistent guidance and support.
- Uphold client confidentiality in accordance with program policies and applicable laws.
- Communicate daily with clients and shelter staff, and check in with the RN at least weekly.
- Assist clients in transitioning out of RCP into the general shelter population or other permanent housing options once medically appropriate.
- Coordinate with shelter staff and homeless services case managers to ensure smooth transitions.
- Monitor and update case plans with defined goals, responsibilities, and timelines based on client needs.
- Communicate regularly with community partners and funding agencies to support client progress and goal achievement.
- Reassess client needs and case management plans periodically, adjusting strategies and goals as necessary.
- Complete all required documentation, including logs, timesheets, reports, and client records.
- Maintain weekly statistical data for each client and submit updates to hospitals and CenCal.
