Job Description:

Job Summary:

Incumbent is responsible for tracking and auditing services to ensure we are meeting quality of care and grant expectations. This individual is the lead on working with any individual with a disability to ensure proper access. Responsible for taking the lead on schedule of shelter coverage

Essential Job Functions:

  • Assist Director in coordination of direct services between all sites.

  • Facilitate weekly team meeting and communicate to director or designee any concerns

  • Manage referrals for service, census list and Master Lists for all sites.

  • Facilitate various psycho-educational groups with residential and outreach clients

  • Chart all pertinent information including individual sessions and document progress toward achieving goals.

  • Provide transportation to residents for goal-related activities.

  • Answer 24-hour hotline and provide crisis support and advocacy as needed. Record calls in Phone Log book.

  • Conduct telephone intakes for women seeking shelter or resources and willing to enter our program.

  • Complete necessary documentation for clients including assessments, consent forms, etc.

  • Provide crisis intervention support to clients.

  • Create and maintain client files.

  • Ensure team memebers has what they need to provide appropriate service to clients (training, supplies etc.)

  • Provide ongoing support to community members and organizations regarding Domestic and Sexual Violence.

  • Provide advocacy including, legal/court advocacy to residential or outreach clients.

  • Track statistics including bed nights, hotline logs and groups.

  • Assist Director with outcomes and reporting.

  • Facilitate ongoing meetings with clients regarding satisfaction with services and coordinate follow up services.

  • Attend meetings in the community to coordinate services for at risk or underserved populations including individuals with disabilities.

  • Provide lead case management services to individuals with disabilities

  • Provide ongoing information during team memebersing regarding individual issues and concerns, as well as clients’ progress towards goals.

  • Maintain a supportive, professional relationships with all team memebers, volunteers, and students.

  • Participate in QI activities.

  • Complete other Duties as assigned by supervisor.

Knowledge, Skills, and Abilities:

  • Basic knowledge of domestic Violence and Sexual Assault.

  • Basic knowledge of trauma

  • Demonstrates good judgment and discernment in dealing with people.

  • Capable of communicating ideas clearly and effectively

  • Demonstrate the ability to operate a computer and operate Google applications.

Experience and Education Qualifications:

  • Bachelor’s Degree in Social Work, Counseling, Psychology, Sociology, or related field and one-year experience in human service organization;

OR

  • Associate’s Degree and two years’ experience in human services field;

OR

  • Three years’ experience providing case management and advocacy pertaining to resource acquisition in a human service organization;

OR

  • A survivor of domestic violence who has demonstrated knowledge and/or experience working in the field of domestic violence and two years’ experience providing case management and advocacy pertaining to resource acquisition.

  • Demonstrated ability to operate a computer and use Microsoft Office applications.

Supervisory Requirements:

  • Communicate the strategic direction of the organization and encourage participation by all team members.

  • Provide leadership and guidance to all aspects of the department.

  • Take an active role in monitoring the identification, development, and execution of strategic objectives.

  • Involve, as appropriate, all team members to achieve goals.

  • Effectively communicate to team members any changes and newsworthy events within the department or company.

  • Handle difficult team member situations directly, using appropriate discretion and Human Resource advice to show respect for the individual

  • Champion change and effectively manage the implementation of new ideas.

  • Reinforces team approach throughout functions; support and solicit input from team members at all levels within the company.

Employment Requirements:

  • Successful completion of background check including criminal record, driving record, abuse/neglect and fingerprint check.

  • Completion of New Hire Orientation at the beginning of employment.

  • All training requirements including Relias at the beginning of employment and annually thereafter.

  • Current driver’s license, acceptable driving record and current auto insurance.

Physical Requirements:

ADA Consideration - Sedentary work: Exerting up to 10 pounds of force occasionally (exists up the 1/3 of the time) and/or a negligible amount of force frequently (exists 1/3 to 2/3 of the time) to lift, carry, push, or pull, or otherwise move objects, including the human body. Repetitive movements of hands, fingers, and arms for typing and/or writing during work shift.

Sedentary work involves sitting most of the time but may involve walking or standing for brief periods of time. Jobs are sedentary if walking and standing are required only occasionally, and all other sedentary criteria are met.
