About the Role
As a Care Coordinator, you will be the heartbeat of our daily operations. You will ensure our clients receive outstanding care and our caregivers feel supported and valued. This position requires someone who is highly organized, a self-starter, and thrives on teamwork and problem-solving.
Responsibilities
Coordinate and manage daily scheduling of in-home care services
Address and resolve client and caregiver concerns promptly and professionally
Answer incoming calls with warmth and efficiency
Maintain accurate care plans and records
Work closely with our care team to ensure seamless service delivery
Participate in an on-call rotation as needed
Requirements
Minimum 3 years of experience in the home care industry
Experience with AxisCare or similar home care scheduling software (preferred)
Strong multitasking, organizational, and problem-solving skills
Excellent communication and teamwork abilities
Comfortable with technology and quick to learn new systems
A self-starter who goes above and beyond for clients and colleagues
