Role Description

This is a full-time on-site role for a Medical Social Worker at St. Joseph Medical Center in Houston, TX. The Medical Social Worker will be responsible for Medical Social Work, Social Work, Discharge Planning, Case Management, and Social Services.

Qualifications

• Medical Social Work and Social Work skills
• Experience in Discharge Planning and Case Management
• Knowledge of Social Services
• Excellent communication and interpersonal skills
• Ability to work effectively in a multidisciplinary team
• Licensed Clinical Social Worker (LCSW) certification is a plus
