Come talk to us about joining the Centerstone team!JOB DESCRIPTION:Duties and Responsibilities: Provide care management services for children, adolescents, adults, and their families who have experienced or witnessed trauma.Duties Include:Coordinate Referrals/linkages to meet individual or youth/family needs (e.g., social services, primary care, juvenile justice services, criminal justice system, employment) and provide follow-up services.Develop, maintain, and utilize community referral sources.Conduct project eligibility and co-occurring disorder screening.Provide referrals for ACA enrollment.Ensure that identified individuals receive needed assistance and support services.Track/support client treatment plan progress, trauma/risk status monitoring, and follow-up.Provide trauma-related education and facilitate preventative care.Provide immediate assistance to clients/families in crisis.Assist veterans/military families in navigating web portals for behavioral health and treatment support; andAttend all meetings, maintain records, and coordinate/consult with community organizations.Complete intake needs assessment and treatment planning.Attend multi-disciplinary team or community at risk meetings on behalf of clients.Qualifications:Bachelor’s degree in social work, psychology, or related field with at least two years’ experience in trauma. Bilingual preferred.Skills and Knowledge Required:Trained in individual trauma issues; experience in child/adolescent trauma preferred. Knowledge of the focus population, able to create a culture of respect, commitment to project goals.Personal Qualities:Must value cultural differences, work independently and/or as a member of a team, problem solve, and be genuinely interested in working with individuals who have experienced and are impacted by trauma.Hours per Week: 40Amount of Travel: 50%Time Type: Full timePay Range: $16.50--$23.10Delivering care that changes people's lives starts with our employees.
