Annual Salary: $75k-$100k (Varies depending on years of experience)

My client is a leading hospice care provider seeking a compassionate Social Worker to join its in-home hospice team. This full-time role focuses on delivering psychosocial support, counseling, and end-of-life planning for patients and their families during one of life’s most sensitive transitions.

The ideal candidate is skilled in counseling, crisis intervention, and care coordination, with experience in hospice, home care, or bereavement services.

Key Responsibilities:

• Conduct psychosocial assessments and provide short-term, goal-directed therapy to patients and families
• Participate in interdisciplinary care planning and support bereavement services
• Assist with referrals, community resource coordination, and end-of-life support
• Maintain thorough and timely documentation of services provided
• Collaborate with staff, volunteers, and external partners to ensure holistic patient care

Qualifications:

• MSW from an accredited program; LMSW or LCSW eligible
• At least 1 year of experience in hospice, home care, or bereavement counseling preferred
• Valid New York State driver’s license
• Strong communication skills and a compassionate, team-oriented approach
