Responsibilities And Duties
- Works every other weekend and on call responsibilities.
- Work with Residents who require assistance with day-to-day living and overseeing the smooth operations of the facility.
- Establishes and maintains positive, respectful and supportive environment for all residents, co-workers, families and facility guests. Assures and welcoming process occurs for all new Residents and co-workers.
- Manages PCA’s. Interviews, hires, orients, coaches, counsels, and evaluates PCA’s.
- Completes job performance evaluations for PCA’s with Wellness Coordinator to sign off.
- Careful monitoring of healthcare routines for residents. Any irregularities must be immediately reported to the Wellness Director who can stabilize the situation.
- Any residents who seem to be in very poor health or at risk for complications must be monitored closely according to a plan created by the resident care coordinator, who interacts with other professionals, residents, and their families. 914
- Performs assessment evaluation for prospective residents
- Works in partnership with the Administrator, Wellness Director, Community Liaison to place them appropriately within the continuum levels of care levels
- Completes RASP required assessments for appropriateness.
- Reviews all DME and RASP required assessments for appropriateness.
- Observes for any signs of resident abuse or neglect. Takes immediate action to prevent harm and takes immediate action to prevent harm and report incidents according to Company policy
- Implementation and training in Eldermark
- Attend and participates in staff meetings, projects, special events, committees, and mandatory training as appropriate
- Maintains safety and health rules by participating in training along with implementing and following protocols related to OSHA & CDC regulations and Cardinal Senior Management Standards as required
- Promotes and encourages individualized resident independence while maintaining resident dignity, individuality, confidentiality and following infection control policies.
- Works as a member of a team to provide essential nursing services and uses effective communication skills.
- High School Diploma
- LPN License highly preferred
- Experience with Microsoft Office
- Successful completion of the PA DPW Direct Care Staff Person Training Course/Test
