We are looking for a phenomenal Hospice Social Worker to join our growing team. In this position you will be providing care in patients' home or facility where they live. As a Hospice Social Worker, you will be providing psychosocial support to patients and families by offering education, support, and resources. As part of the interdisciplinary team, you will be involved in the plan of care as well as assistance of final arrangements and or discharge planning.

Responsibilities:

- Assess the psychosocial dynamics of patients and family/caregivers

- Advocates for the needs of the patient and family

- Has in-depth knowledge and expertise in working with ethnic, cultural and economic diversity

- Ensures the patient's end-of-life wishes are documented and known by assisting with advance directives and other important documentation

- Assists family/caregivers with necessary arrangements and paperwork post mortem

- Contacts local agencies and community resources that may be of help to patients and family/caregivers

Requirements:

- Minimum of 2 years of experience in Hospice

- CPR

- Valid Drivers License and evidence of insurance coverage

Job Type: Full-time

Pay: $68,000.00 - $72,000.00 per year
