Licensed Social Worker ( LICSW)

Key Responsibilities:

• Assist residents and families with psychosocial support and discharge planning

• Coordinate with interdisciplinary team members

• Advocate for resident rights and quality of life

• Maintain accurate and timely documentation

Qualifications:

• Bachelor's Degree in Social Work or related field (BSW required, MSW preferred)

• Previous experience in long-term care or healthcare setting a plus

• Strong communication, problem-solving, and organizational skills

• Knowledge of state and federal regulations
