- Direct Support: Provide hands-on assistance with personal care, daily living activities, and goal achievement for individuals served.
- Case Management: Develop and maintain Individual Support Plans (ISPs), ensuring timely follow-up and delivery of support services.
- Team Leadership: Supervise and mentor staff, evaluate performance, and provide contextual training in therapeutic models.
- Behavioral Support: Design and implement behavior management interventions in collaboration with the clinical team.
- Program Oversight: Act as the site’s contact person in the absence of management, ensuring smooth operations and program integrity.
- Community Engagement: Collaborate with families, guardians, and external agencies to promote individual progress in home, work, and community settings.
Education & Experience:
- Bachelor’s degree preferred.
- 1-3 years of related experience, including staff supervision and case management.
- Valid standard driver’s license (Class C or D).
- Expertise in activity planning, behavior management, and therapeutic techniques.
- Strong understanding of person-centered planning and daily living assistance.
- Ability to foster independence, choice, and growth in a respectful and culturally appropriate manner.
- Familiarity with Seven Hills Foundation policies and procedures and program-specific regulations.
