  • Shift Hours: Monday-Wednesday 8:00 a.m. – 4:30 p.m., Thursday 8:00 a.m. - 7:00 p.m., Friday 8:00 a.m. - 1:00 p.m.

Responsibilities

The Nurse Care Coordinator, RN Responsibilities include :

  • Support care coordination for high-acuity patients
  • Coordinate transportation assistance (bus passes, Uber Health, taxi vouchers)
  • Responsible for medical intake procedures, maintaining daily patient flow, assisting with organization of patient care services, & laboratory and phlebotomy services, and maintaining clinic compliance with governmental agency standards.
  • Provide patient care support through phone calls, walk-in triage, and medication refill requests
  • Coordinate specialist referrals/Appeals and ensure timely follow-up
  • Conduct patient medication reviews and compile diagnostic imaging reports for providers
  • As part of a multidisciplinary team coordinates the provision of all medical services.
  • Assures appropriateness of patient care, facilitates case management, encourages appropriate utilization of resources and timely specialty consults.
  • Complete monthly clinical chart reviews and OTC medication inventory/ordering
  • Provides education to other team members. Perform other duties as assigned.

The Nurse Care Coordinator, RN Requirements

  • RN (Registered Nurse) License
  • Required Associate's or Diploma Nursing
  • Preferred 3 Years of Clinical Related
