Clinical and Crisis Care Coordinator at Oberlin College, serves as an integral member of Oberlin College’s Student Health and Wellbeing team. An ideal candidate understands a strengths-based, ecological approach to ensuring equitable access to mental health care and addressing its related impact on students’ overall well-being.

Reporting to the Director of Counseling and Psychological Services and a dotted line of report to the Assistant Vice President for Student Health and Wellbeing, the position is a 12-month Administrative and Professional Staff (A&PS). The incumbent contributes to providing coordinated, evidence-based, culturally responsive, appropriate, timely and continuity

Clinical Care Coordination (40%)

  • Coordinate care for students who need intensive outpatient or inpatient medical and mental health services, hospital visits, obtaining external referral services for students, maintaining a database of community providers, treatment planning,
  • Cultivate professional relationships with points of contact for outside providers, including providers with specialty areas relevant to student needs and outpatient, intensive outpatient, and inpatient resources.
  • Coordinate follow-up with individuals and, as needed, with consent from students, coordinate community response and resources with Student Support and Outreach Director.
  • Coordinate with a discharge plan, which includes but not limited to obtaining consent and release of information, organize and coordinate with Res Life and keep current familiarity with Oberlin College procedures for health-related voluntary and involuntary hospitalization.
  • Provide clinical case management for students (and their families), faculty and staff seeking guidance around a range of critical and complex student-related issues (i.e., academic, mental/medical health, behavioral and personal).
  • Provide direct service to students (families), faculty and staff seeking guidance around a range of student-related, health, behavioral, and personal issues upon return from hospital visits
  • Provides daily, weekly, or regular check-ins with students who return from hospital visits, with long and short stay at the hospital, especially those who need high level of care, i.e., IOP, PHP to ensure continuity of care,
  • Provides confidential, emotionally supportive interactions with students, parents, administrators, and faculty regarding psychological services. This includes listening to emotional concerns, describing confidentiality and the need for informational releases, suggesting various alternatives, reducing frustration, and resolving problems.

Crisis Care Coordination (30%)

  • Participate in weekly on-call meetings for care coordination, including but not limited to hospitalization and follow ups for acute cases.
  • Supports the triage of clients in identifying the level of care and works with clinicians in connecting clients with the predetermined level of care.
  • Collect and evaluates all facts pertaining to such contacts and formulates appropriate action plans to address concerns
  • Coordinate the College’s process for addressing the needs of students who have crises with academic, family, health, and other issues through providing referrals, follow-up services and when necessary.
  • Provide crisis case management for students interacting with emergency medical services.
  • Coordinate response in partnership with CAPS’ leadership in supporting students in crisis and in need of immediate medical or psychiatric care.

Campus Collaboration (30%)

  • This position is part of a larger multidisciplinary health services team that includes psychiatrists, social workers, therapists, substance use counselors, and psychologists, and student health providers.
  • Engage and maintain continual familiarity with campus resources and regional community referral resources.
  • Under the Administrative Direction of the Assistant Vice President for Student Health and Wellbeing, the Clinical and Crisis Care Coordinator provides data on services utilization that can be shared with VP and Dean of Students and other key stakeholders.
  • Provide regular reports to the Assistant Vice President for Student Health and Wellbeing regarding trends, responsive resources, gaps, and needs.
  • Document student access to off-campus resources and follow through on referral and provision of additional coordination of care, with the office of Student Support and Outreach, and Student Health Services.
  • Collaborate with Health and Counseling Services, Office of Disability and Access, Residence Life, Center for Student Success, Athletic department, Sports Medicine for clinical care coordination, and other appropriate channels to provide care to support students with high clinical needs.
  • Provide organizational support for students transported to and/or admitted to the hospital for medical and mental health reasons.
  • Provide regular updates to all stakeholders involved and coordinate wraparound care and support for students.
  • Develop and maintain familiarity with insurance providers and assist students to navigate coverage.
  • Discuss off-campus referral services, logistics of referral to local providers, and process referral forms including but not limited to the release of information forms.
  • Under the supervision of the Director for Counseling and Psychological Services, the Clinical and Care Coordinator collaborates with outside partners to create and manage MOU’s with local hospitals, care and other entities that serve Oberlin students during physical and mental health crises
  • Collaborates with the Director of Student Support and Outreach, communicates with partners, including families, and Counseling and Psychological Services, Student Health services about current resource availability. Develop individualized referral resources for students and assist to determine availability of initial appointments.
  • Participate in research to elucidate patterns of student departure, analysis of problem areas and recommendation of strategies to reduce student attrition.
  • Perform other duties as assigned.

Required Qualifications:

  • Master’s degree in social work, counseling, or related field.
  • Licensed clinical social worker, clinical professional counselor, or clinical psychologist.
  • Direct experience working in mental health services in an educational setting.
  • Previous employment in mental health, social services, or medical settings.
  • College degree or the equivalent combination of college level coursework and relevant work experience; superior interpersonal and supportive listening skills:
  • Ability to conduct a preliminary assessment of individuals experiencing emotional crises or emergencies; demonstrated ability to deal effectively and efficiently with unpredictable or stressful situations

Desired Qualifications:

  • Previous experience working with young adults, preferably in an educational setting.
  • Experience with crisis assessment, response, and management.
  • Experience providing clinical case management services and a working knowledge of managed care and navigating health insurance.
  • Well-developed counseling and clinical supervision skills.
  • Experience providing culturally responsive services and commitment to on-going anti-racist learning.
  • Knowledge and skills to practice in a manner consistent with professional ethical and legal standards.
  • Ability to maintain documentation of services, consistent with relevant state and federal statutes and regulations.
