Job Title: Master Social Worker (MSW) Spanish/English Speaking Location: New York Hour: Per diem Hourly Rate: $30-35 per hour Company Overview: Priority Cares Home Services is a Minority Business Enterprise (MBE) certified home healthcare service provider specializing in waiver services under the New York Department of Health's Nursing Home Transition and Diversion (NHTD) program and Traumatic Brain Injury (TBI) program. We are committed to delivering high-quality care that promotes independence and community integration for our clients. Position Overview: We are seeking a dedicated MSW (Master of Social Work) to join our team and provide Comprehensive In-Home Care (CIC) and/or Independent Living Skills Training (ILST) services. The MSW will work with individuals directly to develop and implement person-centered plans that support the individual's goals and preferences, and promote independence and community integration. Responsibilities: ▪ Conduct initial and ongoing assessments to identify the individual's needs, preferences, strengths, and challenges. ▪ Develop and implement person-centered plans that address the individual's goals and preferences, and promote independence and community integration. ▪ Provide ongoing support and coaching to individuals in their homes or community settings, as specified in the person-centered plan. ▪ Assist individuals with daily living tasks such as meal preparation, medication management, and personal hygiene. ▪ Provide training to individuals and their families on topics such as budgeting, household management, and communication. ▪ Coordinate with other service providers involved in the individual's care, such as healthcare professionals, therapists, and case managers. ▪ Implement and monitor Positive Behavioral Interventions and Supports (PBIS) strategies, in collaboration with the individual and their team. ▪ Provide Independent Living Skills Training (ILST) to individuals to promote their independence and self-sufficiency. ▪ Maintain accurate and timely documentation of services provided, progress towards goals, and other relevant information. ▪ Attend team meetings and other required trainings and meetings. Maintain confidentiality and adhere to ethical and professional standards. Qualifications: • Bachelor's degree in social work, psychology, or a related field. Experience working with individuals with traumatic brain injuries and/or disabilities. • Knowledge of the NHTD program and regulations. Strong communication and interpersonal skills. • Ability to work independently and as part of a team. • Proficiency in computer skills, including electronic health record systems.
