POSITION SUMMARY:

The Care Coordinator provides field-based and telephonic care navigation services to patients enrolled in the CalAIM Enhanced Care Management (ECM) program provided through San Gorgonio Memorial Hospital. The Care Coordinator builds strong relationships with patients to help them stay engaged in medical care and adhere to their medications. Care Coordinators are committed to removing the patient's barriers to care by identifying critical resources, helping them navigate through health care services and systems, and promoting overall health. They work closely with CalAIM team members and collaborating organizations and providers within their service area.

REQUIREMENTS:

• Education:
  • High school graduate or equivalent preferred.
• Licensure:
  • N/A
• Experience:
  • Bilingual (English/Spanish) Required.
  • At least 2 years of customer service experience preferred.
  • At least 1 years’ experience working with diverse and marginalized populations.
  • Strong understanding of cultural competency with the target population.
  • Computer literacy is desirable.
  • Lived experience that aligns with one or more of the specific DHCS defined Populations of Focus for Enhanced Care Management.
