Summary
Provides transition coordination for CAP/MFP beneficiaries to and from acute care, LTAC, rehabilitation, SNF and home care settings. Maintains a caseload of CAP/MFP beneficiaries and provides whole person interdisciplinary care coordination, and CAP/MFP case management as specified by the Department of Health Benefits (DHB) in Clinical Policy for Community Alternatives Programs.A minimum of one year previous experience in medical social work, case management, disease management transitions coordination, or care coordination is required. Hire date prior to 11/1/2016 Bachelor’s degree in Social Work or related field with 3 years medical case management is accepted. Must maintain State requirements for population served when performing CAP/DA, CAP/C, CAP/Choice or MFP transitions or case management. Demonstrates positive interpersonal skills with excellent communication, problem solving and negotiation skills. Must demonstrate strong organizational skills, critical thinking skills, and the ability to appropriately prioritize direct case management, patient/family support and other duties. Should be knowledgeable of Erickson’s developmental model and be able to demonstrate age specific interventions. Must be knowledgeable of and sensitive to the psychosocial spiritual and cultural needs of individuals. Must be articulate orally and in written communication and be able to make accommodation for literacy and comprehension levels. Must have the ability to use own transportation to make home visits or otherwise carry out duties away from the office.Must have visual acuity being able to see and read medical records documents, computer screens and computer printouts. Speech and auditory abilities are required to allow for effective communication. Must be able to ambulate, to climb stairs and to push or pull wheelchair patients at times. Must have the ability to lift, load/unload client care supplies/equipment up to 30 pounds. Position may involve contact with blood or potentially infectious material.
