Description
Social Worker
The MAAP Social Worker plays a pivotal role in supporting high-risk youth by conducting psychosocial intakes and regular clinical assessments. The position involves working alongside mentors and providing ongoing support through regular home visits and thorough evaluations. The MAAP Social Worker collaborates with families and service providers to ensure comprehensive care, all within a supportive and trauma-informed framework.

Social Worker

Key Responsibilities
COVER LETTER IS REQUIRED
- Conduct psycho-social intake and clinical assessments for referred minors and their households.
- Perform regular home visits to assess safety and risk, ensuring ongoing support and intervention as necessary.
- Collaborate closely with families, educational institutions, and community service providers to create and implement individualized service plans.
- Maintain accurate data entry in ACS and OCFS databases, including ongoing clinical case notes and Family Assessment Service Plans (FASP).
- Develop strong relationships with youth and families, responding promptly to referrals and providing coverage for direct service staff as needed.
Required Skills and Qualifications:
- MSW degree from an accredited institution; LMSW preferred.
- Minimum of three years of experience in youth work and/or youth counseling, with a preference for experience in ACS preventive services.
- Strong understanding of youth development principles and the ability to engage high-risk adolescents through strength-based and trauma-informed practices.
- Excellent relational skills, capable of building trust and rapport with young people and their families.
- Strong organizational, time management, and technical skills; Spanish-speaking ability is a plus but not required.
