Job Overview
We are seeking a dedicated and compassionate Care Manager to join our Enhanced Care Management Team.
Lead Case Manager Professional (e.g., licensed mental health or behavioral
health professional/clinician, social worker, or nurse) or
paraprofessional (with appropriate training and
oversight)
Responsible for coordinating with those individuals and/or entities to
ensure a seamless experience for the member and non-duplication
of services
Engage eligible members
Oversee provision of ECM services and implementation of the care
plan
Offer services where the member lives, seeks care, or finds most
easily accessible and within MCP and state guidelines
Connect member to other social services and supports the member
may need, including transportation
Advocate on behalf of members with health care professionals
Use motivational interviewing, trauma- informed care, and harm-
reduction approaches
Coordinate with hospital staff on discharge plan
Accompany member to office visits, as needed and according to
MCP and state guidelines
Monitor treatment adherence (including medication)
Provide health promotion and self- management training
Care Navigator Paraprofessional, LVN or peer advocate
Navigating support for members
Telephonic and field-based outreach to engage members in care
management program
Establishes close relationships with partners and serves as a point
of contact for members
Provides health education to members to promote self-management
Communicates with care team members on a routine basis to
support care delivery for members
Identify and connect patients to resources for clients to overcome
barriers to care, such as transportation, housing, food, and other
social service resources
Maintain strict confidentiality in accordance with agency policies
Schedule and attend primary care physician appointments to review
and update care plan with the care team
Assist with outreach telephone calls to reduce care gaps and keep
members engaged in care
Provide comprehensive health navigation including assistance with:
o Health insurance enrollment and verification
o Appointment scheduling for providers and specialist
o Appointment scheduling, confirmation, and reminders
o Interpretation and completion of forms and paperwork
o Directing members to community organizations based on
their needs
o Working with clinical teams to review and ensure
understanding of treatment plans
Job Types: Full-time, Part-time, Contract
Pay: $66,000.00 - $74,000.00 per year
